Tips for Completing the 2020-2021 Application
THE APPLICATION MAY BE SUBMITTED TO THE CHILD'S HOME-BASED SCHOOL CAMPUS OR EMAILED TO THAT SCHOOL
Phase 1 applications will be accepted June 1-5, 2020.
Phase 1 eligibility determination letters will be mailed or emailed the week of June 15, 2020.
Phase 2 applications will open June 22 to fill any available seats from Phase 1 and will continue throughout the school year.
Although all documents will be available at the school campuses, they may also be printed and completed at home to minimize the contact on the school campus as we abide by all state guidelines related to the COVID-19 pandemic.
This application is part of the registration process for all programs in the Livingston Parish Early Childhood Community Network:
Livingston Parish Public School Pre-K Classes
Head Start Programs
Type III Child Care Centers

The child's name listed on the application must be the legal name on the child's birth certificate.
Date of Birth: Include the month, day and year
Complete each item on the child's parent(s) / guardian(s)
HOUSEHOLD MEMBERS / FAMILY SIZE:
Include all persons living in the household who are supported by the income of the child's parent(s) or guardian(s).
List the names of all adults and children in the household, their relationship to the child and their date of birth
EMPLOYMENT / INCOME INFORMATION:
The application allows the employment information to be completed on two adults in the household. However, the employment information must be provided for all of the adults in the household, even if there are more than two.
Full time employment is considered to be 40 hours per week.
Hourly wage is recommended to report to determine eligibility
SIGNATURE OF PARENT / GUARDIAN COMPLETING THE APPLICATION AND THE DATE IS REQUIRED AT THE BOTTOM OF THE APPLICATION.
Note: Proof of income on all adults in the household, child’s original birth certificate, child's original social security card, child's current immunization records, and proof of residency must be provided at registration.
Note: If parents are seeking a Pre-K class in a public school, children who qualify will be placed at the school in the district in which the child resides. If that school does not offer a Pre-K class or the class is full, assistance will be given to place the child at another school site.
Seats are not filled first come - first served. The application must be completed in its entirety and all required documents must be submitted by the child's legal guardian making sure it is signed and dated before eligibility will be determined and before it will be entered in the lottery. Eligibility letters will be mailed to all applicants after each site has had the opportunity to offer a Phase 1 registration.
